Assisted Living

The Assisted Living Program at Health Care Group communities offers residents all the conveniences and comfort of the Independent Living Program with the added security of 24-hour assistance and supervision.

Assisted Living provides care for seniors who need daily assistance, but do not require the intensive level of care offered in a skilled nursing facility.

Assisted Living includes a helping hand with day-to-day tasks, including dressing, bathing, personal grooming, dining and medication monitoring.

Assisted Living provides a special combination of residential housing, personalized supportive services and care, while maintaining the dignity of residential living.

ActivCare^® Residential Alzheimer’s Care

The ActivCare® - Residential Alzheimer’s Care program within each Health Care Group community is specially designed to adapt to the unique needs of people who have Alzheimer's disease or other related dementia. The programs are therapeutic in nature and address the physical, mental, psychological and spiritual needs of each individual. The resident's individuality, dignity, and self-esteem are maintained while strengths and abilities are maximized.

The program has flexibility to adapt to each resident needs. For example, a resident who may be up during the night will require more sleep in the morning. Special activities and snacks in the evening hours will be provided to allow the resident to sleep longer in the morning.

The ActivCare® program uses therapeutic activity programming to focus on each individual resident's day-to-day activities. The activities are adult-oriented with assorted topics and themes utilizing reminiscence skills. A daily 24-hour activity schedule may include: exercise, nature walks, arts and crafts, hobbies and activities of daily living. Although there is flexibility for individual participation and they are free to follow their own agenda, residents are encouraged to take part in the organized activities. These carefully planned activities are instrumental in minimizing the restlessness typical to those with Alzheimer's disease and related dementia.

ActivCare® Residential Alzheimer’s Care provides a secure environment with freedom of movement within the unit, as well as outdoors. Resident's individual rooms are decorated with personal items and furniture to ensure a home-like quality. Large living rooms allow for multiple spaces where the activity programs meet the resident's individual preferences. Wide doorways and corridors provide easy access and encourage social interaction.

The ActivCare® program is designed to help residents maintain a high level of self-esteem and dignity through activities and a secure, home-like setting and provides a safe, secure setting for the Alzheimer's resident who does not yet require skilled nursing care.

Our program features:

4. Family involvement. Family members are welcomed and encouraged to participate in daily programs with their loved ones.

Nursing Care, or Skilled Nursing Facilities, are designed for seniors who are in need of 24-hour nursing care. Heath Care Group Skilled Nursing offers an integrated program of medical care and supportive services to strengthen a resident’s physical, mental, emotional, and spiritual well-being.

Whether it’s a Medicare rehabilitative stay, a short-term respite stay or long-term care, our Health Care Centers are committed to personalized skilled nursing care that makes resident and family satisfaction the top priority.

Most Health Care Group communities offer Medicare rehabilitation programs that provide skilled nursing and therapy following a hospital stay. Our multidisciplinary program includes round-the-clock nursing care, rehabilitative therapies, and comprehensive health-related services as well as social services and therapeutic activities administered by compassionate and caring staff.

Skilled Nursing care serves the needs of seniors and their families with personalized 24-hour professional nursing care. Our skilled nursing facilities often serve as rehabilitative environments following surgery or hospitals stays, convalescent settings for individuals with temporary illnesses and secure homes for those who require long-term supervised care.

Our short-term stay option fulfills a variety of needs in the lives of seniors and their family members.

Following an illness or any type of surgery, seniors may need extra assistance to get back in the swing of things. Our respite care program offers special care in the comfort of a private apartment, delicious meals and extra services to assist with getting back on your feet.

There are times when a family member or personal caregiver needs a break or is going on vacation… Our respite care program is perfect for this type of situation.

Many seniors choose a short-term stay when deciding if a permanent move is the right decision for them. Changing environments and moving to a new home is challenging at any age, so we invite seniors to stay with us, experience our community life on a daily basis, and speak to our residents about their experiences, in order to help them make the best decision.

ONE AFFORDABLE DAILY FEE INCLUDES:

Respite care program apartments are private, comfortable and tastefully furnished.

We offer three delicious restaurant-style meals daily, an activity program with a variety of social, educational and fitness programs, a caring and responsive staff with 24/7 availability and emergency call systems in every apartment.

Additionally, housekeeping, linen laundry services are provided, and scheduled local transportation to doctor’s offices, shopping centers and other destinations. Assistance with activities of daily living such as bathing, dressing, personal grooming and medication management are also available for those who need them.

For a small fee, our beauty salon/barbershop which will help you look and feel your best, is conveniently located in the community.

RESPITE STAY REQUIREMENTS:

• Completed physician report
• Current TB test or chest x-ray
• Pre-admission assessment interview completed by a Health Care Group representative
• Doctor’s order (if any) for continued treatment
• Admission agreement and final paperwork signed by guest resident or responsible party
